Precautions and warnings
If you already have high triglyceride levels, or are at risk because of conditions like diabetes, obesity, or drinking a lot of alcohol, you should have your triglyceride levels checked regularly. Get your blood lipids and liver function tested every week or every two weeks. If you have diabetes or might have it, keep an eye on your blood sugar. If you or your family have a history of osteoporosis, bone problems, or metabolic bone diseases, be extra careful. This is also important if you have anorexia nervosa or a history of mental health issues.
This medicine may make it harder to see at night. Do not use wax for hair removal or have skin resurfacing treatments for at least six months. Stay out of the sun and avoid UV light for long periods. Stop taking the medicine if you have hearing loss, stomach pain, bleeding from your rectum, bad diarrhea, or eye problems.
Do not donate blood while taking this medicine and for at least one month after you stop using it.

Drug Interactions
Taking this medication with vitamin A or its derivatives can increase the risk of toxic side effects. It can also make microdosed progesterone birth control less effective, so you should use two forms of contraception. This medication may raise the risk of bone loss when taken with phenytoin, and it may raise your chance of osteoporosis if used with systemic corticosteroids. It can also lower the amount of carbamazepine in your blood.
Warning: Taking this medication with tetracyclines can seriously increase the risk of a dangerous condition called pseudotumor cerebri (increased pressure in the brain).
Mode of actions
Isotretinoin is a man-made vitamin A derivative that makes oil glands smaller and decreases how much oil they produce. It also helps control how skin cells grow and change.
Dosage & Administration
Oral
Acne
Adult: Initially, 0.5-1 mg/kg daily in 2 divided doses. Usual duration of treatment: 15-20 wk; may be discontinued if number of cysts is reduced by >70% (whichever is sooner). Patients intolerant to initial dose: Continue treatment at a lower dose. Patients with very severe acne or acne evident on the body instead of face: Up to 2 mg/kg daily. There should be a 2-mth drug-free interval if a 2nd course is necessary.
Hepatic impairment: Dose reduction may be needed.
Pregnancy & Lactation
This medicine must not be used during pregnancy. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict two effective types of birth control starting one month before and during treatment and for three years after stopping this medicine. Discuss with your doctors if you are not sure which types of birth control are effective for you. You must need to take pregnancy tests regularly during your treatment with this medicine and for at least three years after taking this medicine. If it seems that you do become pregnant stop taking this medicine and call your doctor immediately. If you really become pregnant, miss a menstrual period, or have sex without using two forms of birth control. In some cases, your doctor can prescribe emergency contraception to prevent pregnancy. People on this medicine should not donate blood during the treatment and for three years afterward.
This medicine is also contraindicated breast milk. Breastfeeding is not recommended while using this medication and for three years afterward.
