Existe uma maior probabilidade de níveis baixos de açúcar no sangue quando este medicamento é tomado juntamente com sulfonilureias ou insulina.
Não há razões conhecidas para evitar administrar Antiveneno. No entanto, pessoas que são alérgicas ao soro derivado de cavalo não devem recebê-lo.
Na imunização passiva, antivenenos são injetados no paciente para ajudar a neutralizar o veneno de cobra no sangue.
A dose inicial depende de uma estimativa da quantidade de envenenamento, conforme mencionado abaixo:
1. Envenenamento Mínimo – Edema local progressivo, mas sem sintomas sistêmicos. Recomenda-se a administração de 5 frascos (50ml).
2. Envenenamento Moderado - Estão presentes inchaço além do local da picada, sintomas sistêmicos leves e/ou anormalidades hematológicas e de coagulação. Recomenda-se a administração de 5 a 10 frascos (50 a 100ml).
3. Envenenamento Grave - Efeitos locais extensos e rapidamente progressivos, sintomas sistêmicos e evidência de hemólise ou coagulopatia. Recomenda-se a administração de 10 a 20 frascos (100 -200ml) ou mais.
Em crianças e adultos pequenos (peso corporal inferior a 40 kg), recomenda-se a administração de até 50% mais doses de anti-soro para veneno de cobra.
ADMINISTRAÇÃO:
O anti-soro para veneno de cobra é administrado por via intravenosa não diluído a uma taxa não superior a 1 ml por minuto ou é diluído em 500 ml de fluido intravenoso (injeção de cloreto de sódio ou injeção de dextrose a 5%) e administrado tão rapidamente quanto tolerado durante 1-2 horas. Ao diluir o Snake Venom Antiserum, misture girando suavemente em vez de agitar para evitar espuma.
Infusões adicionais devem ser repetidas de hora em hora até que o inchaço progressivo na parte picada cesse e os sinais e sintomas sistêmicos desapareçam. Quando uma dosagem adequada é alcançada, muitas vezes é observada melhora nos sinais clínicos do paciente.
