Precautions and warnings

**General:**
Metoprolol should be used carefully in people who have liver problems.

**Information for Patients:**
- Take Metoprolol exactly as your doctor tells you, at the same times every day, with or right after food.
- If you miss a dose, just take your next regular dose—do not take two doses at once.
- Do not stop taking Metoprolol without talking to your doctor first.
- Until you know how Metoprolol affects you, avoid driving, using machinery, or doing anything that requires you to be alert.
- Tell your doctor if you have trouble breathing.
- Let your doctor or dentist know you are taking Metoprolol before any surgery.

Drug Interactions

Taking this medication with certain other drugs can increase its effects or cause problems:

- **Catecholamine-depleting drugs (like reserpine) and MAOIs:** Taking these together can make the effects stronger.
- **Sympathomimetics (like some decongestants):** This medication can block the effects of drugs that stimulate alpha1-adrenergic receptors.
- **Cardiac glycosides and some calcium channel blockers (like verapamil or diltiazem):** Taking them together can slow down your heart’s electrical signals even more.
- **Epinephrine:** Sometimes, it can cause the opposite effect than expected when used with this medication.
- **CYP2D6 inhibitors (such as bupropion or cimetidine):** These can increase the amount of medication in your blood.
- **General anaesthetics (like diethyl ether):** There is a higher risk of low blood pressure and heart failure if used together.
- **Vasodilators (like hydralazine) in kidney failure:** This can increase the risk of high blood pressure in the lungs.
- **Rifampicin:** This drug can decrease how much of the medication is in your blood.
- **Antiarrhythmic drugs (like quinidine or amiodarone):** Can increase the medication’s negative effects on the heart’s rhythm and force of contraction.
- **Indomethacin:** May reduce the blood pressure-lowering effect of the medication.
- **Drugs for diabetes:** The medication may increase the effects of medicines that lower blood sugar.

Contraindications

**For High Blood Pressure and Chest Pain (Hypertension and Angina):**
Do not use Metoprolol if you have a slow heartbeat (sinus bradycardia), certain types of heart block (anything worse than first degree), if you are in shock caused by heart problems (cardiogenic shock), or if you have clear signs of heart failure.

**For Heart Attack (Myocardial Infarction):**
Metoprolol should not be given to patients whose heart rate is too slow.

Mode of actions

Metoprolol mainly blocks ?1-adrenergic receptors and has little to no effect on ?2-receptors unless taken in high doses. It does not have membrane-stabilizing effects or act like the body's own adrenaline.

Dosage & Administration

Hypertension: initially 100mg daily, maintenance 100-200mg daily in 1-2 divided doses
Angina Pectoris: 50-100mg 2-3 times daily.
Myocardial Infarction: After early intravenous treatment for 48 hours, patients should receive a maintenance dose of 100mg twice daily.
Arrhythmias: usually 50mg 2-3 times daily; up to 300mg daily in divided doses if necessary.
Hyperthyroidism: (adjunct), 50mg 4 times daily
Migraine Prophylaxis: 100-200mg daily in divided doses.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
