Precautions and warnings

Drug Interactions

Anorexia, tachypnoea, lethargy, metabolic acidosis, coma, and death may occur if used concurrently with high-dose aspirin. Increases phenytoin levels in the blood, increasing the risk of osteomalacia. Primidone concentrations in the blood may be reduced. Other carbonic anhydrase inhibitors have additive effects. Folic acid antagonists' effects may be amplified. Amphetamine and quinidine excretion in the urine may be reduced. Methenamine's urinary antiseptic action may be inhibited. Lithium excretion may be increased. Along with Na bicarbonate, there is an increased risk of renal calculus development. It's possible that ciclosporin levels will rise.

Contraindications

Hepatic insufficiency; hepatic cirrhosis; hyperchloraemic acidosis; severe renal impairment; severe pulmonary obstruction; chronic noncongestive angle-closure glaucoma; adrenocortical insufficiency Lactation and pregnancy.

Mode of actions

Acetazolamide inhibits the enzyme carbonic anhydrase, which catalyzes the reversible reaction involving CO2 hydration and carbonic acid dehydration. It causes alkaline diuresis by increasing the excretion of HCO3 ions as well as Na and K. Carbonic anhydrase is also suppressed in the CNS to prevent neurons from firing abnormally or excessively.

Dosage & Administration

Adult: PO Diuresis 250-375 mg/day.
Glaucoma; Epilepsy 250-1,000 mg/day.
Prophylaxis of high altitude sickness 500-1,000 mg/day.

Child: Glaucoma: 1 mth-12 yr: 10-20 mg/kg daily. Max: 750 mg daily, in 2-4 divided doses. Epilepsy: Neonates and up to 12 yr: Initially, 2.5 mg/kg bid-tid; maintenance: 5-7 mg/kg bid-tid. Max: 750 mg daily.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
