La dexametasona debe usarse con precaución en personas que padecen insuficiencia cardíaca congestiva, presión arterial alta, diabetes, epilepsia, glaucoma, infecciones, insuficiencia renal crónica o niveles elevados de desechos en la sangre (uremia), así como en adultos mayores.
El uso de este medicamento con otros medicamentos que reducen el potasio, como la anfotericina B y los diuréticos de asa, puede aumentar el riesgo de niveles bajos de potasio (hipopotasemia). También puede hacer que medicamentos como la isoniazida, los salicilatos, las vacunas y los toxoides funcionen peor. Cuando se toman junto con dexametasona o ciclosporina, los efectos de estos medicamentos pueden ser más fuertes. Tomar este medicamento con aspirina o alcohol puede aumentar el riesgo de sufrir efectos secundarios estomacales y digestivos.
Advertencia grave: este medicamento puede ser menos eficaz si se toma con efedrina, colestiramina, fenitoína, fenobarbital o rifampicina. Esta combinación puede ser peligrosa.
El herpes simple ocular es una afección en la que nunca se debe utilizar la terapia con corticosteroides. También hay algunas situaciones en las que el uso de corticosteroides debe realizarse con precaución. Estos incluyen tener una úlcera de estómago, infecciones actuales o recientes, huesos débiles (osteoporosis), embarazo, diabetes, problemas renales, presión arterial alta, antecedentes de enfermedad mental, estar a punto de recibir una vacuna o ser alérgico a la dexametasona.
La dexametasona es un esteroide sintético que reduce la inflamación al impedir que los glóbulos blancos se muevan al área afectada y ayudar a que los vasos sanguíneos vuelvan a la normalidad. También reduce la respuesta inmune habitual del cuerpo.
El rango de dosificación oral habitual es de 0,5 a 10 mg al día.
La dexametasona se puede administrar por vía parenteral en dosis de 0,5 a 20 mg al día, ya sea como una única inyección IV/IM o mediante infusión IV.
Niños: 200-500 mcg/kg al día. Edema cerebral: 10 mg IV inicialmente, luego 4 mg IM cada 6 horas según sea necesario durante 2 a 10 días. Se deben administrar grandes dosis intravenosas lentamente para reducir la posibilidad de colapso cardiovascular.
Choque: Por inyección o infusión intravenosa o intramascular 2-6 mg/kg, repetida si es necesario después de 2-6 horas.
La ingesta diaria total de dexametasona, incluso en condiciones agudas, no debe exceder los 80 mg excepto en determinadas circunstancias muy especiales.
Aunque los estudios de reproducción en animales han demostrado un efecto adverso en el feto y no existen estudios adecuados y bien controlados en mujeres embarazadas, durante el embarazo este medicamento se puede utilizar sólo cuando sea definitivamente necesario a pesar de los riesgos potenciales.
Las madres con un bebé recién nacido que han usado este medicamento durante los últimos 3 meses del embarazo rara vez presentan síntomas que incluyen somnolencia, rigidez o temblores musculares, problemas para alimentarse o respirar, y llanto constante. Si nota alguno de estos síntomas en su bebé, especialmente durante el primer mes, informe al médico de inmediato.
Este medicamento pasa a la leche materna. Hable con su médico antes de amamantar.
