Precautions and warnings
डेक्सामेथासोन का उपयोग उन लोगों में सावधानी से किया जाना चाहिए जिन्हें कंजेस्टिव हृदय विफलता, उच्च रक्तचाप, मधुमेह, मिर्गी, ग्लूकोमा, संक्रमण, क्रोनिक किडनी विफलता, या रक्त में अपशिष्ट के उच्च स्तर (यूरेमिया), साथ ही साथ वृद्ध वयस्कों में भी उपयोग किया जाना चाहिए।
Side-effects

Drug Interactions
इस दवा का उपयोग पोटेशियम को कम करने वाली अन्य दवाओं, जैसे एम्फोटेरिसिन बी और लूप डाइयुरेटिक्स के साथ करने से कम पोटेशियम स्तर (हाइपोकैलेमिया) का खतरा बढ़ सकता है। इससे आइसोनियाज़िड, सैलिसिलेट्स, टीके और टॉक्सोइड जैसी दवाएं भी कम काम कर सकती हैं। जब डेक्सामेथासोन या साइक्लोस्पोरिन के साथ लिया जाता है, तो इन दवाओं का प्रभाव अधिक मजबूत हो सकता है। इस दवा को एस्पिरिन या अल्कोहल के साथ लेने से पेट और पाचन संबंधी दुष्प्रभावों का खतरा बढ़ सकता है।
गंभीर चेतावनी: यदि इफेड्रिन, कोलेस्टारामिन, फ़िनाइटोइन, फ़ेनोबार्बिटल या रिफैम्पिसिन के साथ लिया जाए तो यह दवा कम प्रभावी हो सकती है। यह कॉम्बिनेशन खतरनाक हो सकता है.
Contraindications
ऑक्यूलर हर्पीस सिम्प्लेक्स एक ऐसी स्थिति है जहां कॉर्टिकोस्टेरॉइड थेरेपी का उपयोग कभी नहीं किया जाना चाहिए। ऐसी कुछ स्थितियाँ भी हैं जहाँ कॉर्टिकोस्टेरॉइड्स का उपयोग सावधानी से किया जाना चाहिए। इनमें पेट में अल्सर होना, मौजूदा या हालिया संक्रमण, कमजोर हड्डियां (ऑस्टियोपोरोसिस), गर्भावस्था, मधुमेह, गुर्दे की समस्याएं, उच्च रक्तचाप, मानसिक बीमारी का इतिहास, टीका लगने वाला होना या डेक्सामेथासोन से एलर्जी होना शामिल है।
Mode of actions
डेक्सामेथासोन एक मानव निर्मित स्टेरॉयड है जो सफेद रक्त कोशिकाओं को प्रभावित क्षेत्र में जाने से रोककर और रक्त वाहिकाओं को सामान्य स्थिति में लौटने में मदद करके सूजन को कम करता है। यह शरीर की सामान्य प्रतिरक्षा प्रतिक्रिया को भी कम करता है।
Dosage & Administration
सामान्य मौखिक खुराक सीमा प्रतिदिन 0.5-10 मिलीग्राम है।
डेक्सामेथासोन को प्रतिदिन 0.5-20 मिलीग्राम की खुराक पर, एकल IV/IM इंजेक्शन के रूप में या IV जलसेक द्वारा, पैरेन्टेरली दिया जा सकता है।
बच्चा: प्रतिदिन 200-500 एमसीजी/किग्रा. सेरेब्रल एडिमा: शुरुआत में 10 मिलीग्राम IV, फिर 2-10 दिनों के लिए आवश्यकतानुसार हर 6 घंटे में 4 मिलीग्राम आईएम। हृदय पतन की संभावना को कम करने के लिए बड़ी IV खुराकें धीरे-धीरे दी जानी चाहिए।
सदमा: अंतःशिरा या इंट्रामस्क्युलर इंजेक्शन या जलसेक द्वारा 2-6 मिलीग्राम/किग्रा, यदि आवश्यक हो तो 2-6 घंटों के बाद दोहराया जाता है।
कुछ विशेष परिस्थितियों को छोड़कर, गंभीर स्थितियों में भी डेक्सामेथासोन का कुल दैनिक सेवन 80 मिलीग्राम से अधिक नहीं होना चाहिए।
Pregnancy & Lactation
यद्यपि पशु प्रजनन अध्ययनों ने भ्रूण पर प्रतिकूल प्रभाव दिखाया है और गर्भवती महिलाओं में कोई पर्याप्त और अच्छी तरह से नियंत्रित अध्ययन नहीं हैं, लेकिन गर्भावस्था के दौरान, संभावित जोखिमों के बावजूद, इस दवा का उपयोग केवल तभी किया जा सकता है जब इसकी आवश्यकता हो।
जिन माताओं के नवजात शिशु हैं, जिन्होंने गर्भावस्था के आखिरी 3 महीनों के दौरान इस दवा का उपयोग किया है, उनमें शायद ही कभी उनींदापन, मांसपेशियों में अकड़न या कंपकंपी, दूध पीने या सांस लेने में परेशानी, लगातार रोना जैसे लक्षण पैदा हो सकते हैं। यदि आपको विशेष रूप से उनके पहले महीने के दौरान अपने बच्चे में इनमें से कोई भी लक्षण दिखाई देता है, तो तुरंत डॉक्टर को बताएं।
यह दवा स्तन के दूध में पारित हो जाती है। स्तनपान कराने से पहले अपने डॉक्टर से बात करें।
