**Dextromethorphan:**
Dextromethorphan can interact with many medicines, including tricyclic antidepressants (TCAs), antipsychotics, drugs for anxiety and sleep, cimetidine, ciprofloxacin, domperidone, metoclopramide, mexiletine, alcohol, ritonavir, and medicines that block CYP2D6.
**Serious/Fatal Risks:** It can be very dangerous when taken with memantine or moclobemide.

**Triprolidine + Pseudoephedrine:**
Taking these with other stimulant drugs (like other decongestants, TCAs, or appetite suppressants) can raise blood pressure. They can also reduce the effect of blood pressure medicines like methyldopa and beta or alpha blockers. Side effects like drowsiness or agitation may be worse if taken with atomoxetine.
**Serious/Fatal Risks:** There is a serious risk of a dangerous rise in blood pressure (hypertensive crisis) if these are taken with furazolidine or MAO inhibitors (MAOIs)—do not use them together or within 2 weeks of stopping an MAOI. They can also increase the risk of psychosis if taken with bromocriptine.

Contraindications

Patients at risk for respiratory failure, those experiencing an acute attack, people currently taking MAOIs (or who have stopped using them within the past 2 weeks), and those with a long-lasting or persistent cough should use caution.

Mode of actions

Triprolidine is a strong medicine that blocks histamine H1 receptors. It can cause mild drowsiness and has some effects similar to those of drugs that reduce muscle spasms. Pseudoephedrine works by narrowing blood vessels in the nose, making it easier to breathe. It does this by directly stimulating certain receptors (alpha and beta adrenergic receptors) in the respiratory passages. Dextromethorphan is a cough medicine that works by affecting the part of the brain (the medulla) that controls coughing.
