Use with caution in elderly patients, or those with liver or kidney problems, enlarged prostate, angle-closure glaucoma, pheochromocytoma, heart disease, epilepsy, a history of bowel blockage, diabetes, thyroid problems, or urinary retention. It should also be used carefully in people with psychosis, as it may make symptoms worse. If the medicine needs to be stopped, gradually reduce the dose. Watch for suicidal thoughts, especially at the beginning of treatment. Use during pregnancy and breastfeeding only if clearly needed.
There is a higher risk of serotonin syndrome if you take this with SSRIs, SNRIs, TCAs, triptans, fentanyl, lithium, tramadol, tryptophan, or buspirone. It can also make blood pressure medicines like bethanidine, guanethidine, debrisoquine, and clonidine less effective. Taking it with barbiturates may make your body break down the drug faster. Using it with anesthesia increases the chances of heart rhythm problems and low blood pressure. Fluoxetine can raise the drug’s level in your blood.
**Dangerous Interactions:** Do not use this with MAOIs, linezolid, or methylene blue, as it can cause serotonin syndrome, which may be life-threatening.
Mania, recent heart attack, abnormal heart rhythms (especially heart block), severe liver problems, and use in children.
Nortriptyline is a type of antidepressant known as a tricyclic, and it comes from the breakdown of another drug called amitriptyline. It works by raising the levels of certain brain chemicals, serotonin and norepinephrine, by stopping their reabsorption in the brain. This helps improve mood and relieve symptoms of depression.
Oral
Depression
Adult: 75-100 mg/day in 3 or 4 divided doses. Max: 150 mg/day in severe depression.
Child: Adolescent: 30-50 mg/day in divided doses.
Elderly: 30-50 mg/day in divided doses.
Nocturnal enuresis
Child: 6-7 yr (20-25 kg): 10 mg; 8-11 yr (25-35kg): 10-20 mg; >11 yr (35-54 kg): 25-35 mg. All doses are given 30 minutes before bedtime and treatment should continue for not >3 mth.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
