Extra caution is needed for those with kidney or liver problems, those having electroconvulsive therapy, or those with low stomach acid or high stomach pH because this can lower how well the medicine is absorbed. This medication should be used with care during pregnancy and breastfeeding. Young adults may have a higher risk of suicidal thoughts or behavior, so they should be closely monitored. The medicine can make it unsafe to drive or do other tasks that need full attention. Do not stop taking it suddenly.
- Cyproheptadine and phenytoin can reduce the levels or effects of this drug.
- Carbamazepine, cimetidine, and medicines that block CYP2D6 (like chlorpromazine, delavirdine, fluoxetine, miconazole, pergolide, quinidine, quinine, ritonavir, and ropinirole) can increase its levels or effects.
- This drug can increase the levels or effects of atomoxetine, carvedilol, clozapine, certain CYP2B6 drugs (such as bupropion, promethazine, propofol, selegiline, sertraline), certain CYP2D6 drugs (such as amphetamines, some beta-blockers, dextromethorphan, fluoxetine, lidocaine, mirtazapine, nefazodone, risperidone, ritonavir, thioridazine, tricyclic antidepressants, venlafaxine), duloxetine, galantamine, mexiletine, pimozide, procyclidine, and propafenone.
- It decreases the effects of certain painkillers that are CYP2D6 prodrugs (like codeine, hydrocodone, oxycodone, tramadol).
- It slows down the breakdown of dextromethorphan, haloperidol, and thioridazine.
- It can make the slowing of the heart rate from beta-blockers worse.
- It increases the harmful effects if taken with other medicines that depress the central nervous system (CNS).
- There’s a higher risk of serotonin syndrome if taken with amphetamines, SSRIs, meperidine, nefazodone, trazodone, serotonin agonists, sibutramine, sympathomimetics, tramadol, or venlafaxine.
- It increases bleeding risk when combined with NSAIDs, aspirin, warfarin, or other blood thinners.
- It can make people more sensitive to amphetamines.
- It can cause nerve damage if taken with lithium.
- It can increase low blood sodium risk if used with loop diuretics.
- Mania or high blood pressure can happen if taken with selegiline.
- **Dangerous:** Serious, possibly fatal, reactions can happen if used with nonselective MAO inhibitors (MAOIs).
Do not use this product if you are currently taking MAOIs, or within 14 days of stopping them. Also, do not use it together with thioridazine or pimozide.
Paroxetine mainly works by blocking the reabsorption of serotonin in the brain. It doesn’t have much effect on other neurotransmitter sites, such as muscarinic receptors.
Adult: PO Depression Initial: 20 mg/day. May increase if needed. Max: 50 mg/day.
Obsessive-compulsive disorder Initial: 20 mg/day. May increase if needed. Maintenance: 40-60 mg/day.
Panic disorder Initial: 10 mg. May increase if needed. Maintenance: 40-60 mg/day.
Social anxiety disorder Initial: 20 mg/day. May increase if needed. Max: 50-60 mg/day.
Anxiety; Posttraumatic stress disorder Initial: 20 mg/day. May increase if needed. Max: 50 mg/day. Premenstrual dysmorphic disorder As HCI: Extended release Initial: 12.5 mg once daily. May increase if needed. Take throughout menstrual cycle or limited to the luteal phase.
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
