Precautions and warnings

Trifluoperazine should be used with caution in people with heart disease, epilepsy, angle-closure glaucoma, exposure to very hot or cold temperatures, the elderly, Parkinson's disease, myasthenia gravis, enlarged prostate (benign prostatic hyperplasia), diabetes, and those with kidney or liver problems. Stop taking trifluoperazine at least 48 hours before a myelography exam and do not start again until at least 24 hours after the procedure. Do not use trifluoperazine to control nausea or vomiting before or after myelography if metrizamide is used. Also, avoid using during pregnancy.

Drug Interactions

Taking this medicine with other drugs that slow down the brain, like opiates, painkillers, sleeping pills, anesthesia, or alcohol, can make drowsiness worse. There’s a higher risk of side effects if you use it with medicines that have antimuscarinic effects, such as some antidepressants (TCAs) or drugs for Parkinson’s disease. This medicine can reduce the effectiveness of dopaminergic drugs like levodopa. It may also increase the risk of low blood pressure if taken with blood pressure medicines or trazodone. It can block the blood pressure-lowering effect of guanethidine. When used with lithium, it may increase the risk of serious movement problems or nerve toxicity. Lastly, antacids can lower how much of this medicine your body absorbs.

Contraindications

Do not use if you have existing central nervous system (CNS) depression or are in a coma; have bone marrow problems, blood disorders, liver disease, or are allergic to phenothiazines; or if you have tumors that depend on prolactin. Not recommended during the first trimester of pregnancy or while breastfeeding.

Mode of actions

Trifluoperazine works by blocking dopamine D2 receptors in the brain. It doesn’t cause much drowsiness or anticholinergic side effects, but it can strongly cause movement-related side effects (extrapyramidal symptoms) and helps prevent nausea and vomiting. It is mainly used to manage severe agitation or violent behavior, but it can also be used to treat anxiety that is not related to psychosis.

Dosage & Administration

Adult: PO Psychoses 2-5 mg twice daily, up to 15-20 mg/day. Severe or resistant: 40 mg/day.
Nausea and vomiting; Short-term management of anxiety 1-2 mg twice daily. Max: 6 mg/day.
For anxiety: Max duration: 12 wk.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
