Precautions and warnings

Low blood pressure, weak heart function, or heart failure (which can get worse), severe narrowing of the aortic valve, liver problems, diabetes, porphyria, and pregnancy are all conditions to be cautious with. Do not stop taking this medication suddenly, as this can make chest pain (angina) worse. If chest pain due to lack of blood flow to the heart occurs after taking the medication, stop using it.

Drug Interactions

The blood pressure-lowering effects are stronger when this drug is taken with other antihypertensives, aldesleukin, or antipsychotics. If used with fentanyl during surgery, it can cause dangerously low blood pressure. This drug may affect how your body responds to insulin and glucose. When taken with benazepril, it reduces the expected fast heart rate effect. Taking it with coumarin blood thinners may increase your risk of bleeding. The drug’s levels in your blood can go up if you use medications that block CYP3A4 (such as some antifungals, cimetidine, erythromycin, HIV-protease inhibitors, nefazodone, fluoxetine, or quinupristin/dalfopristin).

Danger: CYP3A4 inducers (such as rifampicin, phenytoin, or carbamazepine) can lower this drug’s effects and make it less effective.

Contraindications

Used for:
- Acute heart attack (MI)
- Cardiogenic shock (a serious condition where the heart can’t pump enough blood)
- Sudden, unstable chest pain (unstable angina)
- Treating chest pain during a chronic (long-term) heart condition (chronic stable angina)

Mode of actions

Nifedipine works by blocking calcium ions from entering the heart and smooth muscle cells during depolarization. This action helps to relax and widen blood vessels (both in the body and in the heart), lowering blood pressure and reducing resistance in the arteries (afterload). It also increases blood flow to the heart. However, the body may respond by increasing heart rate (reflex tachycardia). Nifedipine does not usually affect the way electrical signals travel through the heart and rarely weakens the strength of heart contractions.

Dosage & Administration

Adult: PO HTN Extended release 10-40 mg twice daily or 20-90 mg once daily. Angina pectoris Extended release 10-40 mg twice daily or 30-90 mg once daily. Liquid-filled cap: 5-20 mg 3 times/day. Raynaud's syndrome Liquid-filled cap: 5-20 mg 3 times/day.

Pregnancy & Lactation

Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.

Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.

This medicine passes into breast milk. Talk to your doctor before breastfeeding.
