Drug Interactions
Obat-obatan yang mempengaruhi sistem saraf pusat (seperti barbiturat, fenotiazin, antidepresan, dan MAOI) dapat meningkatkan efeknya pada otak jika dikonsumsi bersamaan. Skopolamin mungkin membuat Anda lebih mengantuk dan juga bisa menyebabkan halusinasi atau perilaku tidak biasa. Kafein, teofilin, atau aminofilin dapat membuat obat penenang dan pereda kecemasan menjadi kurang efektif. Mengonsumsi skopolamin dengan natrium valproat atau probenesid dapat meningkatkan jumlah obat dalam darah Anda.
Contraindications
Masalah hati yang serius, kesulitan bernapas, glaukoma sudut sempit akut, dan sedang hamil atau menyusui.
Mode of actions
Lorazepam adalah obat kerja cepat dari keluarga benzodiazepin. Ia bekerja dengan menempel pada reseptor spesifik pada sel saraf di otak, terutama di area seperti sistem limbik dan formasi retikuler. Tindakan ini meningkatkan aliran ion klorida ke dalam sel, yang meningkatkan efek menenangkan dari bahan kimia otak yang disebut GABA. Hasilnya, sel-sel saraf menjadi lebih stabil dan kecil kemungkinannya untuk menjadi terlalu aktif.
Dosage & Administration
Dewasa: PO Anxiety 1-6 mg/hari dalam 2 atau 3 dosis terbagi, maksimal 10 mg/hari.
Insomnia berhubungan dengan kecemasan 1-4 mg/hari sebelum tidur.
Profilaksis mual dan muntah yang berhubungan dengan terapi sitotoksik Untuk kemoterapi emetogenik sedang: Dapat ditambahkan 1-2 mg pada terapi antiemetik.
Premed pada pembedahan 2-3 mg pada malam sebelum operasi, kemudian dosis yang lebih kecil pada pagi hari jika diperlukan.
Untuk pasien lanjut usia atau pasien yang lemah, dianjurkan dosis awal 1 hingga 2 mg/hari dalam dosis terbagi, untuk disesuaikan sesuai kebutuhan dan dapat ditoleransi.
IV Status epileptikus 4 mg, ulangi sekali setelah 10-15 menit jika diperlukan.
Sedasi pada perawatan kritis 0,02-0,06 mg/kg setiap 2-6 jam sebagai injeksi atau 0,01-0,1 mg/kg/jam sebagai infus kontinu.
IV/IM Kecemasan akut 25-30 mcg/kg setiap 6 jam jika diperlukan.
Premed pada pembedahan 50 mcg/kgBB, diberikan 30-45 menit sebelum pembedahan bila diberikan melalui inj IV atau 1-1.5 jam sebelum pembedahan bila diberikan melalui inj IM.
Pregnancy & Lactation
Obat ini tidak diperbolehkan selama kehamilan. Jangan gunakan jika Anda sedang hamil. Dapat diserap melalui kulit dan paru-paru sehingga dapat merusak organ tubuh anak Anda yang belum lahir dan menyebabkan cacat bawaan. Alat kontrasepsi yang efektif dan ketat selama pengobatan dan selama enam bulan setelah menghentikan obat ini. Jika Anda hamil, hentikan penggunaan obat ini dan segera diskusikan dengan dokter Anda.
Belum ada informasi spesifik apakah obat ini masuk ke dalam ASI atau tidak. Mengingat kemungkinan risiko pada anak-anak, menyusui saat menggunakan obat ini tidak dianjurkan dan setidaknya dua minggu setelah dosis terakhir. Konsultasikan dengan dokter Anda sebelum menyusui.
