Precautions and warnings

Drug Interactions

Medications that affect the central nervous system (like barbiturates, phenothiazines, antidepressants, and MAOIs) can increase the effects on the brain when taken together. Scopolamine might make you more sleepy and could also cause hallucinations or unusual behavior. Caffeine, theophylline, or aminophylline may make sedative and anxiety-reducing medications less effective. Taking scopolamine with sodium valproate or probenecid may increase the amount of the drug in your blood.

Contraindications

Serious liver problems, trouble breathing, acute narrow-angle glaucoma, and being pregnant or breastfeeding.

Mode of actions

Lorazepam is a fast-acting medication from the benzodiazepine family. It works by attaching to specific receptors on nerve cells in the brain, especially in areas like the limbic system and reticular formation. This action increases the flow of chloride ions into the cells, which boosts the calming effects of a brain chemical called GABA. As a result, the nerve cells become more stable and less likely to become overactive.

Dosage & Administration

Adult: PO Anxiety 1-6 mg/day in 2 or 3 divided doses, up to 10 mg/day.
Insomnia associated w/ anxiety 1-4 mg/day at bedtime.
Prophylaxis of nausea and vomiting associated with cytotoxic therapy For moderately emetogenic chemotherapy: May add 1-2 mg to the antiemetic therapy.
Premed in surgery 2-3 mg the night before operation, then a smaller dose in the morning if needed.
For elderly or debilitated patients, an initial dosage of 1 to 2 mg/day in divided doses is recommended, to be adjusted as needed and tolerated.

IV Status epilepticus 4 mg, repeat once after 10-15 mins if needed.
Sedation in critical care 0.02-0.06 mg/kg 2-6 hrly as inj or 0.01-0.1 mg/kg/hr as continuous infusion.

IV/IM Acute anxiety 25-30 mcg/kg 6 hrly if needed.
Premed in surgery 50 mcg/kg, to be given 30-45 minutes before the operation if given via IV inj or 1-1.5 hr before operation if given via IM inj.

Pregnancy & Lactation

This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.

There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
