Bupropion should be used very carefully in people with a history of seizures or with conditions that increase the risk of seizures, such as severe liver disease, brain tumors, or those who are suddenly stopping alcohol or benzodiazepines. People with other risk factors for seizures—like alcohol abuse, past head injuries, diabetes, or who are taking drugs that can cause seizures—should only take bupropion if it is truly necessary. It should also be used with caution in people with bipolar disorder, psychosis, recent heart attacks, unstable heart disease, liver or kidney problems.
Neuroleptics, lithium, tricyclic antidepressants (TCAs), benzodiazepines, alcohol, and other drugs that lower the seizure threshold can increase side effects when taken with levodopa. Fluoxetine can reduce how quickly the liver clears these drugs. Be cautious when giving this medication with other drugs that affect liver enzymes. Using it with ritonavir can raise the risk of toxic effects.
**Warning:** Taking this drug together with MAO inhibitors can cause severe toxicity and may be life-threatening.
Bupropion Hydrochloride should not be used in people who have seizure disorders, because it can increase the chance of having a seizure, and this risk goes up with higher doses. It should also not be used with other medications that contain bupropion. People who have or have had bulimia or anorexia nervosa should not take bupropion, since studies show they are more likely to have seizures when using it. Bupropion should never be taken at the same time as a monoamine oxidase inhibitor (MAOI); you need to wait at least 14 days after stopping an MAOI before starting bupropion. Lastly, anyone who has had an allergic reaction to bupropion or any of its ingredients should not take this medication.
Bupropion HCl slightly blocks the reuptake of norepinephrine, serotonin, and dopamine in the brain. It is thought to help people quit smoking mainly because of its effects on norepinephrine and dopamine.
Oral
Smoking cessation
Adult: Modified-release preparation: Initially, 150 mg once daily for 6 days then increased to 150 mg bid. Period of treatment: 7-12 wk. To discontinue treatment if abstinence is not achieved by 7th wk. Max: 300 mg/day.
Elderly: 150 mg/day.
Depression
Adult: Initially, 100 mg bid increased to 100 mg tid after 3 days if necessary. Increased further to 150 mg tid if no improvement has been observed after several wk of therapy. Max: 150 mg tid. As a modified-release preparation: 150 mg once daily in the morning, increased to 150 mg bid after 3 days if necessary, may further increase to 200 mg bid after several wk if needed. Max: 450 mg as a single dose.
Although animal reproduction studies have shown an adverse effect on the fetus and there are no adequate and well-controlled studies in pregnant women, but during pregnancy, this medicine can be used just when definitely required despite potential risks.
Mothers with a newborn baby who have used this drug during the last 3 months of pregnancy may seldom create symptoms including drowsiness, muscle stiffness or shakiness, feeding or breathing troubles, constant crying. If you notice any of these symptoms of your baby's particularly during their first month, tell the doctor immediately.
This medicine passes into breast milk. Talk to your doctor before breastfeeding.
