Precautions and warnings
يجب على الأشخاص الذين يعانون من الحالات التالية توخي الحذر: مشاكل في القلب، الجلوكوما ضيقة الزاوية، صعوبة في التبول، تاريخ من الصرع، مشاكل في الكلى أو الكبد، الخضوع للعلاج بالصدمات الكهربائية، انخفاض ضغط الدم، فرط نشاط الغدة الدرقية أو تناول دواء الغدة الدرقية، الأفكار الانتحارية، أولئك الذين هم على وشك إجراء عملية جراحية، النساء الحوامل أو المرضعات، أولئك الذين يقومون بعمل يحتاج إلى التركيز الذهني، وكبار السن. لا تتوقف فجأة عن تناول هذا الدواء.
Side-effects
- جفاف الفم. اضطرابات في التبول. النعاس وزيادة التعرق. العجز الجنسي. ارتباك، تنمل، ترنح، رعاش؛ أعراض خارج الهرمية. طنين الأذن، والدوخة، والتعب، والصداع. زيادة الوزن عند النساء؛ التثدي وثر اللبن. قد تكون مميتة: الوفاة، نادرة (باستثناء المرضى الذين يعانون من إحصار كبير في القلب والمرضى الذين يتلقون علاج MAOI). تحريض الهوس لدى الأفراد الذين يعانون من مرض الهوس الاكتئابي أو تفاقم الذهان لدى الأفراد الذهانيين بالفعل.
Drug Interactions
تجعل الباربيتورات الجسم يتحلل مضادات الاكتئاب ثلاثية الحلقات بشكل أسرع، في حين أن الأدوية مثل السيميتيدين والجوانيثيدين والهالوبيريدول والفينوثيازين تبطئ تحللها. تكون تأثيرات الكحول على الدماغ أقوى إذا تم استخدامها معًا.
**تحذير خطير:** في حالة التحول من مثبطات MAOI إلى كلوميبرامين، انتظر 3 أسابيع على الأقل بعد إيقاف مثبطات MAOI. يمكن أن يؤدي استخدام الأدرينالين أو النورادرينالين مع الأدوية ثلاثية الحلقات إلى زيادة خطر ارتفاع ضغط الدم وعدم انتظام ضربات القلب.
Contraindications
لا تستخدم هذا الدواء إذا كان لديك حساسية منه. تجنب استخدامه في نفس الوقت مع MAOIs (نوع من الأدوية). لا ينبغي استخدامه أثناء التعافي بعد نوبة قلبية، أو عند الأشخاص الذين يعانون من إحصار القلب أو عدم انتظام ضربات القلب، أو أثناء نوبات الهوس، أو عند الأطفال.
Mode of actions
يمنع كلوميبرامين بقوة إعادة امتصاص السيروتونين في الدماغ. كما أنه يمنع بشكل كبير المستقبلات الكولينية وألفا -1، بينما يؤثر بشكل ضعيف على مستقبلات الدوبامين. يحتوي هذا الدواء على تأثيرات مضادة للاكتئاب ومهدئة (مهدئة) ومضادة للكولين.
Dosage & Administration
للبالغين: مساعد في حالة الجمدة المرتبطة بالخدار الأولي: 10 مجم/يوم، حتى 75 مجم/يوم.
اضطراب الوسواس القهري؛ اضطراب الهلع؛ الرهاب الأولي: 25 مجم/يوم، يمكن زيادته تدريجياً حتى 100-150 مجم/يوم إذا لزم الأمر. الحد الأقصى: 250 ملغ/يوم.
الاكتئاب الأولي: 10 ملغ/يوم، حتى 30-150 ملغ/يوم إذا لزم الأمر. > قد تكون هناك حاجة إلى 250 ملغ / يوم في الحالات الشديدة.
Pregnancy & Lactation
على الرغم من أن دراسات التكاثر الحيواني أظهرت تأثيرًا سلبيًا على الجنين ولا توجد دراسات كافية ومراقبة بشكل جيد عند النساء الحوامل، إلا أنه خلال فترة الحمل، يمكن استخدام هذا الدواء فقط عندما تكون هناك حاجة إليه بالتأكيد على الرغم من المخاطر المحتملة.
نادراً ما تظهر على الأمهات اللاتي لديهن طفل حديث الولادة والذين استخدموا هذا الدواء خلال الأشهر الثلاثة الأخيرة من الحمل أعراض بما في ذلك النعاس، وتصلب العضلات أو الارتعاش، ومشاكل التغذية أو التنفس، والبكاء المستمر. إذا لاحظت أياً من هذه الأعراض لدى طفلك خاصة خلال شهره الأول، أخبري الطبيب فوراً.
ينتقل هذا الدواء إلى حليب الثدي. تحدثي مع طبيبك قبل الرضاعة الطبيعية.
