Medicamentos que afetam o sistema nervoso central (como barbitúricos, fenotiazinas, antidepressivos e IMAOs) podem aumentar os efeitos no cérebro quando tomados em conjunto. A escopolamina pode deixá-lo mais sonolento e também causar alucinações ou comportamento incomum. Cafeína, teofilina ou aminofilina podem tornar os medicamentos sedativos e redutores da ansiedade menos eficazes. Tomar escopolamina com valproato de sódio ou probenecida pode aumentar a quantidade do medicamento no sangue.
Problemas hepáticos graves, dificuldade em respirar, glaucoma agudo de ângulo estreito e gravidez ou amamentação.
Lorazepam é um medicamento de ação rápida da família dos benzodiazepínicos. Funciona ligando-se a receptores específicos nas células nervosas do cérebro, especialmente em áreas como o sistema límbico e a formação reticular. Esta ação aumenta o fluxo de íons cloreto nas células, o que aumenta os efeitos calmantes de uma substância química cerebral chamada GABA. Como resultado, as células nervosas tornam-se mais estáveis e menos propensas a se tornarem hiperativas.
Adulto: Ansiedade PO 1-6 mg/dia em 2 ou 3 doses divididas, até 10 mg/dia.
Insônia associada à ansiedade 1-4 mg/dia ao deitar.
Profilaxia de náuseas e vómitos associados à terapêutica citotóxica. Para quimioterapia moderadamente emetogénica: Pode adicionar 1-2 mg à terapêutica antiemética.
Premed em cirurgia 2-3 mg na noite anterior à operação, depois uma dose menor pela manhã, se necessário.
Para pacientes idosos ou debilitados, recomenda-se uma dose inicial de 1 a 2 mg/dia em doses divididas, a ser ajustada conforme necessário e tolerada.
IV Status epilepticus 4 mg, repetir uma vez após 10-15 minutos, se necessário.
Sedação em cuidados intensivos 0,02-0,06 mg/kg 2-6 horas como injeção ou 0,01-0,1 mg/kg/h como infusão contínua.
IV/IM Ansiedade aguda 25-30 mcg/kg 6 horas se necessário.
Premed em cirurgia 50 mcg/kg, a ser administrado 30-45 minutos antes da operação, se administrado por injeção intravenosa, ou 1-1,5 horas antes da operação, se administrado por injeção IM.
Este medicamento não é permitido durante a gravidez. Não use se estiver grávida. Pode ser absorvido pela pele e pelos pulmões, podendo danificar os órgãos do feto e causar deficiências congênitas. Controle de natalidade rigoroso e eficaz durante o tratamento e por seis meses após a interrupção do medicamento. Se engravidar, pare de tomar este medicamento e fale com o seu médico imediatamente.
Não há informações específicas sobre se este medicamento passa ou não para o leite materno. Considerando os possíveis riscos para as crianças, não é recomendada a amamentação durante o uso deste medicamento e pelo menos duas semanas após a última dose. Consulte seu médico antes de amamentar.
