Precautions and warnings
Imipramine should be used carefully and under close supervision from a doctor, especially in older adults and those with enlarged prostate, trouble urinating, or glaucoma—especially angle-closure glaucoma. Its sedative (calming) effects become stronger if taken with other substances that slow down the brain, like alcohol, sleeping pills, other sedatives, or antihistamines. Imipramine can also speed up the heart and put extra strain on it, making it risky for people with heart problems—especially anyone who recently had a heart attack. Older adults and people with heart disease may be at risk for irregular heartbeats, heart failure, heart attacks, fast heart rates, or strokes. People starting imipramine need to be closely monitored for suicidal thoughts until the right dose is found, as the risk of suicide can be higher—especially if imipramine is taken in large amounts or mixed with alcohol. Starting imipramine can sometimes trigger manic episodes or make symptoms of existing mental health problems worse.
Side-effects

- loss of appetite
- diarrhea
- and abdominal cramping are all side effects associated with imipramine usage in a small number of people.
Drug Interactions
Plasma levels of the drug can increase if taken with quinidine, cimetidine, SSRIs, propafenone, or flecainide. Barbiturates and phenytoin can lower its plasma levels. The drug may also make anticholinergic medications stronger. Taking it with altretamine can cause severe drops in blood pressure when standing. Mixing it with alcohol can lead to drowsiness and affect your ability to perform tasks.
**Dangerous Reactions:**
Using this drug with adrenaline, noradrenaline, or methylphenidate may cause life-threatening high blood pressure. It can weaken the blood pressure-lowering effect of drugs like guanethidine, bethanidine, debrisoquine, bretylium, methyldopa, and clonidine. Taking it with MAOIs (monoamine oxidase inhibitors) might cause serotonin syndrome; there should be a 3-week gap between these medications.
Contraindications
Imipramine should not be taken at the same time as a MAO inhibitor, or within 14 days of stopping a MAO inhibitor. Taking both together can cause serious problems such as very high blood pressure, extreme restlessness, high fever, muscle stiffness, severe seizures, coma, or even death.
Imipramine should not be used in people with serious liver or kidney problems, a history of blood disorders, or anyone who is allergic to imipramine or similar tricyclic antidepressants (dibenzazepine group). It is also not safe for people who are recovering from a recent heart attack, or for those with seizure disorders or glaucoma.
Mode of actions
Imipramine mainly blocks the re-uptake of noradrenaline and, to a lesser degree, also blocks the re-uptake of serotonin.
Dosage & Administration
Depression, initially up to 75 mg daily in divided doses increased gradually to 150-200 mg (up to 300 mg in hospital patients); up to 150 mg may be given as a single dose at bedtime; elderly initially 10 mg daily, increased gradually to 30-50 mg daily; child not recommended for depression.
Nocturnal enuresis, child 7-8 years 25 mg, 8-11 years 25-50 mg, over 11 years 50-75 mg at bedtime; max. period of treatment (including gradual withdrawal) 3 months-full physical examination before further course.
Pregnancy & Lactation
This medicine is not allowed during pregnancy. Do not use it if you are pregnant. It can be absorbed through the skin and lungs caused can damage your unborn child’s organs and cause congenital disabilities. Strict effective birth control during treatment and for six months after stopping this medicine. If you do become pregnant, stop taking this medicine and discuss your doctor right away.
There is no specific information on whether this medicine passes into breast milk or not. Considering the possible risks for the children, breastfeeding while using this medicine is not recommended and at least two weeks after the last dose. Consult your doctor before breastfeeding.
